Game Show Participation & Social Media Consent Agreement

By submitting this form and participating in any Chaz Carti Enterprises production, you agree to the following terms:

1. Consent to Be Recorded

I understand and agree that I may be photographed, filmed, and audio recorded during participation in the game show, interviews, behind-the-scenes footage, promotional content, and related activities.

2. Social Media & Promotional Use

I grant permission to Chaz Carti Enterprises, its affiliates, production team, and media partners to use my name, likeness, voice, appearance, video footage, and photographs for promotional, marketing, advertising, entertainment, and social media purposes across all platforms including, but not limited to:

Instagram
TikTok
YouTube
Facebook
Websites
Streaming Platforms
Paid Advertisements

3. No Compensation

I understand that participation is voluntary and that I will not receive compensation, royalties, or ownership rights for any media captured unless otherwise agreed upon in writing.

4. Public Distribution

I acknowledge that content may be publicly distributed online and may be viewed, shared, reposted, or commented on by the public.

5. Release of Liability

I release and hold harmless Chaz Carti Enterprises, its owners, staff, videographers, editors, contractors, and affiliates from any claims, liabilities, damages, or legal actions related to the use of recorded media and participation in the production.

6. Participant Confirmation

By signing below, I confirm that:

I am at least 18 years old
I voluntarily agree to participate
I have read and understood this agreement
I consent to the use of my image and recordings as stated above
